1. Tell the doctor or nurse about any health issues you or your child has and any medications you are taking including any refills you may need.

2. Please provide our staff with as much information as possible in case we need to reach you:

• Other

• If you have internet access, please create an account for each family member who has been cared for by Cook County Health at https://cookcountyhealth.org/patients-visitors/mycookcountyhealth/.
