Register for the online Computer Training course within 60 days prior to the start of your rotation. Complete the following steps to register:

As proof, please print the Infection Control certificate, and the email from us with your passing score for the Rotators Orientation test (we grade it and email you the score – you must receive a passing score of 8/8 or 100%).

Documentation – Required documents needed at the time of check-in with our office prior to the start of your rotation are as follows (you must bring with you – DO NOT fax or email):

Current medical License

Valid NPI Number

Copy of your current ACLS or PALS card (ALL PGY Levels- no programs excluded)

Completed Immunization Form & TB or Quant Gold with record of titers

Flu Vaccine Verification (administered with-in current Flu season)

Copy of ECFMG certificate (if foreign medical graduate)

Copy of Professional Liability Insurance, Background Check and Drug Screen (if your residency does NOT have a standing agreement with Cook County Health)

Proof of Radiation Safety Training (if rotating in ORTHOPEDICS, CARDIOLOGY or UROLOGY)

Copy of your Block Rotation Schedule (must show your Cook County Health Rotation start & end dates)
***Once you have completed collecting the requirements above, please visit our NEW office location at least THREE but no more than TEN business days prior to your rotation start date.***

PLEASE UNDERSTAND THAT WE CANNOT ISSUE YOU AN ID BADGE, COMPUTER ACCESS AND PHARMACY PRIVILEGES IF YOU HAVE NOT MET ALL OF THE REQUIREMENTS.
NOTICE:
It is the responsibility of the rotating resident to return the ID Badge/Security Card within 5 days of completing the rotation at Cook County Hospital (if year-long access – return ID at end of academic year):

If the card is lost or stolen notify your institutional coordinator immediately

There is a $50 charge to replace the card

If you fail to return the card your institutional coordinator will be notified and a Fee will be sent to your institution
