Juvenile Justice Care Coordination

Cook County Health’s (CCH) Juvenile Justice Care Coordination (JJCC) team works to support justice involved youth in navigating their care across systems and providers, connecting youth to CCH and community based services, and addressing other areas of need related to social determinants of health.

Referrals come from various sources, including youth and family requests, Juvenile Probation, the State’s Attorney Office, Cook County’s Juvenile Temporary Detention Center, the Illinois Department of Juvenile Justice, law enforcement, identified medical homes, and community providers.

Youth engage in screening and assessment to allow for coordination of services in the community to address identified needs, meet with their JJCC team for regular case management calls, and participate in Individual Care Team meetings to insure effective communication between referring parties, families, and providers.

The goal of care coordination is to reduce barriers that interfere with the ability to interface with community based providers through the use of education and supportive services and linking referred youth to behavioral health and other community based programs (mentoring, vocational programs).

JJCC also works to assist families of justice involved youth with enrollment in Medicaid and other benefits, and meet with youth and families in the community, in their homes, and at identified provider locations to best aid in successful utilization of the health system.

The JJCC team works to connect youth and families to a variety of providers and community resources. We aim to connect our members with supports to address all of their goals. As such, we coordinate care with a variety of community based partners to address a range of service needs, including:

  • Mental health counseling
  • Psychotropic medications
  • Substance use
  • Trauma therapy
  • Mentoring
  • Work force development
  • Educational supports
  • Primary care connections
  • Recreational activities

We believe every youth and every family has their own unique recipe for success. Through regular case calls to our members, care team meetings with community partners and the member’s support team, and individualized care plans, the JJCC team helps every family find the right fit. 

Who and How to Refer?
JJCC serves justice involved youth and families who are at risk for increased involvement with multiple systems (juvenile justice, child welfare, behavioral health, etc.). Youth who are appropriate for referral are residents of Cook County, between the ages of 10 and 21 years old, who meet at least 1 of the following criteria:

  • Recent inpatient hospitalization or assessment by SASS
  • Current or historical psychotropic medications
  • Identified need for outpatient counseling to address symptoms of mental health issues, substance use, or trauma symptoms
  • Previous mental health or substance use diagnosis

Chicago Beyond’s investment in Storycatchers is an opportunity to break the cycle of reincarceration within Chicago’s juvenile justice system. Chicago Beyond is working with Storycatchers to grow the impact of their post-release employment program, Changing Voices. Previously a part-time program, with the support of Chicago Beyond, Changing Voices now runs full-time, and empowers young people to write their true stories. These resilient youth turn narratives, often influenced by traumatic life-experiences, into powerful musical theatre. They perform these stories for the community and facilitate discussions to create change in themselves and their audience. The program ultimately prepares them for meaningful education and/or career-oriented employment.
