Call (312) 864-0200 to make, change or cancel an appointment. You can also call us at that number to access care coordination, for dental services, or for questions about referrals. Our appointment line is open Monday through Friday, 8:00 AM to 6:00 PM, to help you find the right appointment and answer any questions you may have.
Cook County Health ExpressCare offers virtual immediate care visits from 7:00 AM - 7:00 PM, Monday through Friday. Weekend hours of operation coming soon. This service is available for adults and children of all ages. Start a virtual immediate care visit: https://cchvirtualcare.org
Use the MyCookCountyHealth Patient Portal to message with your doctor, view some lab results and future appointment dates, and ask for medication renewal.

What is the cancellation and rescheduling policy?
If you need to cancel or reschedule your appointment, please give us at least 24 hours’ notice. You can do this online through the patient portal or by calling our appointment line.

How can I access my medical records?
