Cook County Health Opens Fifth Large-Scale Community Vaccination Site in Forest Park to All Eligible Illinois Residents

Illinois Governor JB Pritzker joined Cook County Board President Toni Preckwinkle, Cook County Health CEO Israel Rocha Jr. and other elected officials to announce that Cook County has opened its fifth large-scale vaccine administration site at 7630 West Roosevelt Road in Forest Park, Illinois.

“Cook County is committed to getting vaccines into the arms of residents who want them,” said Cook County Board President Toni Preckwinkle. “There is no doubt that Cook County and some of its most vulnerable communities have been disproportionately affected by the COVID-19 pandemic, and to get to the other side of these historic times, we need to continue vaccinating as many people as possible. Having mass community vaccination sites like this one is the only way we will beat this virus.”

Vaccinations at Forest Park and other Cook County Health sites are by appointment only. Cook County Health will continue to only offer appointment slots based on the amount of vaccines expected and will add appointments as new vaccine shipments arrive.

Individuals can be make an appointment at vaccine.cookcountyil.gov. Individuals without internet access or who need assistance scheduling can call 833-308-1988 (from 7 a.m. to 7 p.m., Monday – Friday).

At this time, the Forest Park vaccination site will administer the Pfizer vaccine. When fully operationalized and with ample vaccine supply, Cook County Health estimates it will be able to provide close to 4,900 vaccines daily at this new location.

“Cook County Health’s new Forest Park community vaccination site will give us the ability to vaccinate the greatest number of people in one, centralized location,” said Cook County Health CEO Israel Rocha Jr. “While vaccine supply is increasing across the county and here locally, it is still in very high demand, so we ask everyone to be patient.”

In addition to Cook County Health sites, the Cook County Department of Public Health has distributed vaccines to more than 90 partner locations including Jewel-Osco, Mariano’s, Walgreens, Federally Qualified Health Centers and hospitals who are also offering vaccines by appointment. Additional sites can be found at coronavirus.illinois.gov/s/vaccination-location.

Residents not yet eligible for vaccine can also sign up at vaccine.cookcountyil.gov to receive updates on their phase and for information on dozens of additional vaccination sites across Cook County.
