Common Services

Cook County Health (CCH) understands the challenges you and your family face as you try to navigate mental health issues and types of supports available. We are here to help!

We provide a broad range of services for adults experiencing mental health disorders and substance use issues. Our clinics offer personalized treatment plans that include both medication management and therapy.

Around half of US adults have or will have a mental health condition at some point in their lives. It’s important to know you are not alone.

Through our Integrated Behavioral Health programming at the 13 Cook County Health Ambulatory Community Health Network (ACHN) centers, we offer outpatient mental health therapy and case management services.  Our Integrated Behavioral Health team is made up of highly trained and experienced clinical psychologists, licensed clinical social workers, licensed professional counselors and masters level case managers.  This team works collaboratively with your primary care team to create direct access to high quality mental health support and treatments such as comprehensive assessment and treatment planning, individual, family and group therapies as well as linkage to other care and supports within cook county health or other community providers as needed. Therapists work collaboratively with individuals to develop patient-centered treatment goals that are unique to each patient.

Talk to your primary care physician or call your local ACHN site to access outpatient mental health services at CCH.

We extend our services to young individuals, offering specialized care tailored to the unique needs of children and adolescents.

Psychiatric services at CCH are provided by Board certified psychiatrists and psychiatric nurse practitioners.  Services are patient-centered, recovery focused and are typically part of a collaborative care approach.  You may be referred to other outpatient supports such as individual psychotherapy, group therapy, and case management services as needed. Medication Assisted Treatment (MAT) and Recovery Support programs.

At Cook County Health our Substance Use treatment and Recovery services are designed to support patients in achieving their individual recovery goals and provide whole-person, person-centered care to improve the physical, mental and social well-being. Taking a harm-reduction based approach, CCH prioritizes helping individuals improve their quality of life and reduce deaths related to opioid overdose.  Patients of our MAT program have access to a team to support their recovery journey.

The team consists of three primary team members:

  • Primary Care provider—prescribes medication for addiction treatment and integrated primary care.
  • Recovery Coach-a team member with direct or indirect lived experience with substance use recovery who can provide education, support the identification of individual recovery goals and creating a plan to help achieve them.
  • Licensed clinical therapist- supports individuals with co-occurring substance use and mental health including anxiety, depression, trauma, etc.
