How to Make an Appointment
Phone Scheduling

Call (312) 864-0200 to make, change or cancel an appointment. You can also call us at that number to access care coordination, for dental services, or for questions about referrals. Our appointment line is open Monday through Friday, 8:00 AM to 6:00 PM, to help you find the right appointment and answer any questions you may have.

Cook County Health ExpressCare offers virtual immediate care visits from 11:00 AM - 7:00 PM, Monday through Friday. Weekend hours of operation coming soon. This service is available for adults and children of all ages. Start a virual immediate care visit: https://cchvirtualcare.org/

Use the MyCookCountyHealth Patient Portal to message with your doctor, view some lab results and future appointment dates, and ask for medication renewal.

Is my insurance accepted?

We accept a wide range of insurance plans. Click here to review a full list of the insurance plans accepted. If you have questions about whether your insurance is accepted or need information on payment options, please visit our Billing & Insurance page.

If you need to cancel or reschedule your appointment, please give us at least 24 hours’ notice. You can do this online through the patient portal or by calling our appointment line.

  1. Insurance Card: Bring your current insurance information.
  2. ID: A valid photo ID.
  3. Medical Records: Any relevant medical records or referrals from other providers.
  4. Medication List: A list of medications you’re currently taking.

Forms: Complete any required forms before your appointment. These can be downloaded [through your patient portal/from our forms page/picked up at our office].

Fasting: If your appointment requires fasting (e.g., for blood tests), please follow the specific instructions provided by our office.

We recommend scheduling as early as possible to secure your preferred time. Appointment availability may vary depending on the service and provider.

Yes, you can request a specific provider when scheduling your appointment. If they are available, we will do our best to accommodate your request.

If you’re running late, please call us as soon as possible. Depending on the time and circumstances, you may need to reschedule your appointment.
