Upcoming Appointments and Prescriptions

You may have upcoming appointments at Stroger Hospital, or another Cook County Health location. To check any upcoming appointments or to schedule an appointment please call 312-864-0200.

When you are released, a 30-day supply of your prescription medications is sent to be filled at Stroger Hospital Pharmacy, 1969 W. Ogden Avenue, Room 1130 (1st floor), Chicago, IL 60612. To check on the status of a prescription, please call 312-864-1608.

Drug & Alcohol Use Care

If you were receiving Methadone / Suboxone / Vivitrol for drug or alcohol use treatment, or would like to start, we want to help with your care plan. To contact your care coordinator, call 773-674-0634 or 708-738-6917 (cell).

Methadone: If you have a new clinic, they can call the Cermak Health Services Pharmacy within Cook County Jail at 773-674-5622 or 773-674-5623 to confirm your last dose.

Vivitrol: Your care coordinator can get you an appointment for your next dose.

Buprenorphine (Suboxone): Your care coordinator can make your next appointment for you and request your next prescription to be filled at a pharmacy near you.

Sexually Transmitted Diseases

Cook County Health’s CORE Center
2020 W. Harrison Street, Chicago, IL, provides walk in screening for sexually transmitted diseases, including HIV and hepatitis.

You can also get a PrEP prescription from their on-site pharmacy. Walk-in hours are Monday-Friday 8:30am-4:30pm. Call 312-572-4500 to make an appointment or ask a question.




CountyCare

You may have enrolled in Medicaid during intake. If you did, watch your mail  for your Medicaid card. For more information about State of Illinois benefits, including Medicaid or SNAP food benefits, please visit www.abe.illinois.gov or call 1-800-843-6154.

If you have health coverage through CountyCare, a Medicaid managed care health plan, you have access to many free resources to help you get and stay healthy.

CCH Holiday Hours

Stroger Hospital and Provident Hospital are open 24 hours a day, 7 days a week, including holidays. If you are experiencing a medical emergency, please dial 911, or go to your nearest emergency department.

CCH Health Centers will be open limited hours for the holidays.

Thanksgiving Day – Thursday, November 28, 2024: CLOSED
