On Wednesday, November 17, 2021, Cook County Health hosted a ribbon cutting at its new Belmont Cragin Health Center at 5501 West Fullerton Avenue, Chicago.

The new $12 million, 25,000-square-foot health center offers pediatric and adult atención primaria y la salud de la mujer, as well as specialty care services such as cardiología y endocrinología. Las mujeres, infantes y niños (WIC) food assistance program is also on-site, and COVID-19 vaccines are available to both existing patients and members of the community.

“All too often, your ZIP code and your race are direct determinants in your access to high-quality health care – and the country is waking up to this terrible fact,” Cook County Board President Toni Preckwinkle said. “Our collective health as a county is dependent on everyone having access to health care centers that are close to their home and offer a wide range of services. This is exactly the kind of care we will be providing at the Belmont Cragin Health Center; this is equity in action.”

The Belmont Cragin Health Center replaces CCH’s Logan Square facility. It is twice the size of the Logan Square site and is equipped with 30 exam rooms compared to 12 at Logan Square. The health center can accommodate more than 37,000 visits annually.

“The pandemic has shone a light on the importance of having access to health care that is close to home,” said CCH CEO Israel Rocha. “Cook County Health has been prioritizing projects that will allow us to provide primary and specialty care to our patients where and when they need it. The availability and accessibility of medical care is essential to ensuring our communities’ long-term health and well-being.”

This health center is one part of CCH’s broader health equity strategy to provide high-quality, patient-centered care in the neighborhoods where its patients live and work.

Increasing access to care with a focus on individual and community wellness helps reduce unnecessary health care costs, and decrease emergency room visits and inpatient stays, ultimately improving the health of patients in these communities.

The center’s patient demographics are reflective of the diverse and dynamic communities in the area. The health center serves patients of all backgrounds. There is a comprehensive patient care team that includes bilingual Spanish-speaking staff members to better serve patients in the surrounding neighborhoods.
