住院 姑息治疗 咨询服务： Fellows will spend up to 5 months on the inpatient consultation services divided between Rush University Medical Center and Stroger Hospital of Cook County. The consultation services at both institutions have well established interdisciplinary teams and are clinically intensive rotations.
住院临终关怀部： Fellows will rotate in a hospice inpatient setting dedicated to high acuity support for hospice patients and their families and for the care of the imminently dying. The fellows will spend a total of 4 weeks in this interdisciplinary setting.
Home Hospice: Fellows will complete a total of 8 weeks of training with Unity Hospice and Palliative Care in home-based hospice and palliative care settings. During this rotation, fellows will gain experience working collaboratively with the interdisciplinary hospice team and caring for patients and families in the community setting.
家庭和长期护理： Fellows will spend at least 4 weeks seeing patients at home and in long-term care settings, focusing on facilitating inpatient-outpatient continuity of care, especially as related to the practice of palliative and hospice medicine.

Seema Kapur MD, FACS, comes to the CCH hospice and palliative care fellowship program with an interest in patient and care giver support during recovery, chronic illness, and end of life, that is borne out of years of practice as a general surgeon, as well as impactful personal experiences. She immigrated to the US after high school in India and completed her bachelor's in biology and medical degree at the University of Illinois at Chicago. For residency she went to Saint Joseph Mercy Hospital in Ann Arbor returning there as teaching faculty and full-time staff, after fellowship training in minimally invasive techniques and robotics at Swedish Medical Center in Seattle. Outside of work she enjoys hiking, cooking with family, and gardening.
Zita Chan is an incoming Hospice and Palliative Medicine fellow interested in end-stage lung disease and ICU outcomes. She is from Chicago and went to medical school at UIC. She is excited to return to the Illinois Medical District and work with everyone!
Yusra A. Jangda is excited to join the Palliative Care fellowship program at Cook County/Rush University. Originally from Toronto, Canada, she completed her family medicine residency training in Minneapolis, MN and decided to stay in the Midwest for fellowship! Loving the versatility of family medicine, the acuity of cases, complex relationships, and dynamics of chronic disease management drew her into further training in Palliative Medicine. Outside of medicine, she loves a walkable city, hiking and being outdoors, travelling, Solidcore, and indulging in a sweet treat.
