To begin your weight management therapy, please visit your Cook County Health primary care provider (PCP) for a proper assessment and treatment plan. Your PCP can offer treatment options and refer you to services to treat:

  • Obesity

Bariatric Surgery Program 

Bariatric surgery, also known as weight loss surgery, is a type of surgery that helps individuals living with obesity to lose weight and improve their health. It involves making changes to the stomach, and in some cases, the intestines, to help a person eat less and feel full more quickly. This surgery can be a way for people who have struggled with losing weight through diet and exercise alone to get the help they need to live healthier lives.

Cook County Health’s multidisciplinary team of doctors, nurses, dietitians, physician assistants and psychologists will work with you throughout the process before and after surgery.

Who Qualifies for Bariatric Surgery?

  • Body Mass Index (BMI): Individuals with a BMI of 35 or higher or those with a BMI of at least 30 along with obesity-related health issues such as type 2 diabetes, high blood pressure, sleep apnea, fatty liver disease and GERD.
  • Age: You must be 18 years of age or older.
  • Psychological Evaluation: An assessment of mental health is required to ensure that candidates are emotionally prepared for the lifestyle changes required after surgery.
  • Medical Evaluation: A thorough medical examination helps determine if a candidate is fit for surgery and can manage potential risks. This process will take at least 6 months before a surgery date is offered.
  • Commitment to Lifestyle Change: Candidates must be willing to commit to a post-surgery lifestyle that includes healthy eating habits and regular exercise. As part of your surgical preparation, you will also have the opportunity to work alongside a dietitian.

The Lifestyle Center

Cook County Health is committed to empowering those we serve to adopt healthier lifestyle behaviors. CCH has multiple options throughout our system for patients to connect with lifestyle experts such as registered dietitians, physical therapists and behavioral counselors.

At CCH’s Lifestyle Centers, patients can access education on healthy eating, exercise, and stress management for those needing chronic disease and/or weight management.

To connect to our Lifestyle Center or dietitian services, schedule an appointment with one of our Primary Care Providers and request a referral.

Food As Medicine

Cook County Health believes access to nutritious food is essential for achieving health equity. Through our Food As Medicine initiative (FAM), we connect patients to food resources that support long-term wellness.

Food As Medicine uses nutrition and food choices to prevent, manage, or even treat chronic diseases and promote overall health. Cook County Health’s Food As Medicine team partners with clinical staff to integrate healthy eating and proper nutrition into medical care to improve outcomes, reduce healthcare costs, and empower individuals to take control of their health.

The FAM initiative has developed a guide outlining CCH-approved insurance plans. This guide provides detailed information about the potential benefits available to support patients' nutritional and food security needs, as well as instructions on how to access these resources.
