CountyCare’s innovative rewards program empowers members to earn financial incentives for preventive care, advancing members’ health and wellness, plus financial flexibility to buy essentials for themselves and their families.

CountyCare members collectively spent $14.8 million in rewards dollars in 2024 through the CountyCare Rewards Program. Half of the reward dollars were used to purchase food,

This milestone highlights the growing impact of the program, which encourages members to engage in preventive care and wellness activities to earn rewards. Reward dollars can be used to buy a range of essential goods and services, enhancing members’ overall well-being.

“This program is just one of the ways CountyCare works hard to support its members,” said Toni Preckwinkle, Cook County Board President. “Not only do rewards encourage people to be healthy, but they also help reduce everyday financial stress.”

“Going to the doctor and staying on top of your care isn’t always easy but is essential to living your healthiest life. The Rewards Program recognizes the time and effort it takes for members to be engaged in care. By investing in our members’ health and wellness, we can help ensure they are supported in both their healthcare journey and their daily lives,” said Dr. Erik Mikaitis, Cook County Health CEO.

CountyCare members receive a CountyCare Visa Rewards Card that is loaded with cash credit when they take part in certain health activities. For example, a member can earn $50 for going to their annual visit with a primary care doctor, $50 for getting a mammogram, $25 for managing their diabetes, or $10 for pediatric immunizations. Members can continue to earn rewards throughout the year, with more cash credit added to their cards as they complete additional health services.

The CountyCare Visa Rewards Card can be used anywhere Visa is accepted, including a wide variety of retailers, such as grocery stores, gas stations, restaurants, pharmacies, and more. The card cannot be used to purchase alcohol, tobacco, or firearms.

In 2024, over 360,000 CountyCare members had more than 800,000 transactions with their CountyCare Visa Rewards Cards. More than $7 million, or about 50% of the rewards dollars spent in 2024, were used for food, more than $1.5 million was used for fuel or transportation, and $580,000 to pay for other utilities.

“This incredible milestone reflects the dedication of our members to their health and the value they place on the support CountyCare provides,” said Aaron Galeener, CountyCare Chief Administrative Officer. “We are thrilled that our Rewards Program is helping members improve their lives by offering them financial flexibility to meet every day needs, whether it’s paying for a utility bill, getting groceries, or covering transportation costs.”

CountyCare is a no-cost health plan and offers a wide range of rewards. Members can also qualify for extra benefits like car seats, Sleep Safe Kits, transportation to medical appointments, and food assistance through CountyCare.

CountyCare recently earned a four-star rating by the National Committee for Quality Assurance (NCQA), making it one of the highest-quality Medicaid health plans for Cook County residents enrolled in HealthChoice Illinois, the State’s Medicaid managed care program. CountyCare is the largest Medicaid managed care plan serving Cook County residents with more than 400,000 members. The plan is owned and operated by Cook County Health, one of the nation’s leading public health systems.
