We are posting this notice as part of Cook County Health’s commitment to patient privacy.

On February 7, 2019, the Office of Corporate Compliance was notified that a workforce member improperly obtained and viewed medication information for some individuals who received care from Cermak Health Services. Through our investigation, we learned that this incident may have impacted individuals who received care between September 27, 2018 and February 19, 2019. Unfortunately, certain information, known as Protected Health Information, was included on the medication information. This included individual names, medical record numbers, billing numbers, and types of medication. We apologize for this.  Importantly, we are not aware of any misuse of this data. 

Since this occurrence, an investigation was performed and we are continuing to make every effort to ensure this does not happen again. These efforts are called corrective actions and include:

  • Reviewing the circumstances to help us prevent a similar occurrence in the future.
  • Discussing the event with the staff member involved. 
  • Providing disciplinary action for the staff member involved.
  • Putting in place a plan to ensure everyone is aware of our commitment to our patients’ privacy.

We know our patients trust us with confidential information and we have a responsibility to maintain that trust through our actions. All of us, within Cook County Health, apologize for this privacy breach. We recognize we must and will do everything we can to protect our patients’ privacy.
